Continuum of Care

Recovery Continues Beyond a Single Program

Treatment needs can change throughout recovery. Someone who initially requires a highly structured level of care may need different services and supports as they become medically stable, develop recovery skills, and return to everyday responsibilities.

Meridian provides a continuum of treatment and recovery services designed to support individuals through different stages of that process.

Our continuum includes detoxification and withdrawal management, residential treatment, outpatient services, medication treatment, family support, peer recovery services, discharge and aftercare planning, and alumni connection.

Planning for the next stage of recovery begins during treatment. Our team works with clients to identify continued clinical care, recovery supports, community resources, and other services that can help maintain stability and build on the progress made at Meridian.

The goal is not simply to complete treatment. It is to leave treatment better prepared to continue recovery.
