Individualized Care for the Whole Person

No two recovery journeys are exactly alike. Meridian Health Services develops care around each person’s strengths, needs, experiences, and goals at each level of care.


At Meridian we recognize that substance use can affect a persons physical health, emotional well-being, relationships, housing, employment, legal concerns, and daily functioning. Treatment is designed to address the whole person—not only substance use.

Evidence-Based & Trauma-Informed

Our clinical team uses evidence-based and trauma-informed practices to help individuals understand patterns, develop coping skills, strengthen motivation, and prepare for continued recovery.



Services may include individual and group counseling, recovery education, relapse prevention, peer support, family programming, care coordination, medication treatment, and aftercare planning, tailored to the individuals needs.

Integrated Support

Substance use disorders and mental health concerns often occur together. When appropriate, our teams consider both as part of assessment, treatment planning, and care coordination.

A Collaborative Treatment Process

Individuals are encouraged to participate actively in treatment planning and goal setting. Our teams communicate across disciplines and, with proper authorization, may coordinate with families, healthcare professionals, referral partners, and community supports.

Continuity Beyond a Single Program

Recovery continues after a person completes one level of care. Discharge and transition planning help connect individuals with appropriate next steps, ongoing services, recovery resources, and community support.
