Referral Information

Meridian Health Services accepts inquiries and referrals from individuals, families, hospitals, healthcare providers, behavioral health organizations, courts, probation and parole departments, employers, Employee Assistance Programs, recovery organizations, and other community partners.

To begin a referral, call 248-599-8999 x1. Please be prepared to provide:


  • The individual’s name, date of birth, and contact information
  • The requested service or reason for referral
  • Current substance use and immediate safety or withdrawal concerns
  • Relevant medical and mental health information
  • Insurance or funding information, if available
  • Referral-source contact information
  • Any required assessment, court, discharge, or medical documentation


We protect the privacy of individuals seeking or receiving substance use disorder treatment. A referral source may provide information to help initiate care, but Meridian may need the individual’s participation and written authorization before sharing information about screening, admission, attendance, treatment, or discharge.


For hospitals and professionals: Calling early can help the admissions team identify required records, authorization steps, transportation considerations, and realistic timing before discharge or transfer.
