Documentation & Reporting Requirements

The Secretary of State requires specific documentation as part of the reinstatement process.

  • Meridian Health Services helps ensure all materials are properly prepared, including:
  • Completed Substance Use Evaluation (state-compliant format)
  • 12-panel urine drug screen results
  • Verified support group attendance (if applicable)
  • Letters of support that meet AHS standards


We guide you through what is required and help identify any missing or incomplete items before submission.

Drug Screening Requirement

A 12-panel urine drug screen is required by the Michigan Secretary of State as part of the evaluation process.



Meridian Health Services will provide a referral for this screening at no additional cost during your evaluation process.
