- Meridian Health Services helps ensure all materials are properly prepared, including:
- Completed Substance Use Evaluation (state-compliant format)
- 12-panel urine drug screen results
- Verified support group attendance (if applicable)
- Letters of support that meet AHS standards

Drug Screening Requirement
A 12-panel urine drug screen is required by the Michigan Secretary of State as part of the evaluation process.
Meridian Health Services will provide a referral for this screening at no additional cost during your evaluation process.
